An employee was taking blank temperatures on a forming machine. As the employee inserted blank temperature probes into the blanks, their right index finger was pinched between the blank arm and the operating shaft. The finger was degloved to the first knuckle, and was later medically amputated.

An employee was troubleshooting a new machine. A piece of metal fell from the machine and struck the employee's left middle finger, causing an amputation.

An employees thumb tip was crushed and amputated in a machine.

An employee was taking the temperature of a mold inside a foaming machine and dropped the temperature probe. As the employee tried to retrieve the probe, the left index finger was amputated.

An employee was operating a band saw when his right little fingertip was amputated.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was operating a trim saw. While trying to free a jammed board she had just cut, she came into contact with the running saw blade, which amputated her right thumb.

An employee was using a shaper to cut wood when their right hand contacted the blade, resulting in a partial amputation to the right ring finger.

On September 1, 2024, two employees were dislodging the latch of a hopper to dump a load of glass fragments. The injured employee stopped to walk around to the other side. The other employee freed the latch and the load of glass fragments was dumped, pinning the injured employee to the ground. The injured employee was hospitalized with a fractured left femur.

An employee was walking when they fell and struck the front of their head.

An employee was adjusting the vacuum control valve on the right side of an A-frame arm when their finger was caught between the vent blow pipe and the control valve. The employee's right middle finger was partially amputated.

An employee was opening the milling machine when they began experiencing respiratory issues such as a running rose, coughing, and eye itching from the milling machine vapors. The employee was hospitalized.

An employee was rolling a table with glassware product loaded on it. The table collapsed onto the employee, who suffered a broken lower leg.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
