An employee was riding a horse when it saw or smelled a mountain lion and spooked, bucking twice. On the first buck, the employee's face smashed into the crown of the horse's head. On the second buck, the employee was thrown to the ground. The employee suffered blunt force trauma to the facial area, including a broken jaw, sinus damage, and three broken bones near the right eye. The employee also sustained a broken right wrist upon hitting the ground.

An employee was on a horse performing ranch work when the horse went into a ditch and fell over on the employee. They suffered hip and leg injuries.

An employee was trying to rope the rear legs of a cow. The cow changed direction and the rope whipped, causing the employee to fall from his horse. He landed on the ground on his back and broke several ribs.

An employee was riding a horse to check for sick cattle when the horse became spooked and bucked the employee off. The employee was knocked unconscious and suffered a brain bleed, fractured ribs, fractured vertebrae, and a tear in his left shoulder.

An employee was participating in a training when they were thrown from a horse. The employee struck the ground and sustained a concussion.

An employee was on break in a maintenance shed when a propane space heater caused their clothing to catch on fire, resulting in second- and third-degree burns to their lower back, buttocks, and upper leg.

Camp American Legion

On October 2, 2024, an employee was loading a plastic table (84" x 18") into the bed of a golf cart from the back. The table slid forward and depressed the gas pedal, releasing the parking brake. The golf cart reversed and pinned the employee between the cart and the front doors of the recreation center. The employee was hospitalized with fractured ribs and a ruptured spleen.

An employee was participating in a skit in the dining hall and was supposed to run into the window as part of the skit. The window broke and the employee fell through the second-story window to the ground below. The employee sustained a fracture to their spinal canal.

An employee was riding on the back of a jet ski on a lake with two team members. The jet ski was stopped and when the driver started to accelerate, the employee fell backward off the vehicle. The jet ski's water exhaust ruptured their skin and injured their internal organs, resulting in hospitalization.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
