An employee walked into a co-worker's cubicle to answer the phone. To access the phone, the employee positioned herself between the cubicle desk and chair. This position placed the employee's feet under the desk. As she exited the cubicle, her left foot became entangled with a satchel strap that was beneath the cubicle desk. She then tripped and fell to the floor, suffering a right hip femoral fracture.

An employee was descending a 30-inch step stool when they lost their footing and fell to the ground. The employee sustained a fracture of their upper right tibia.
An employee was on the ground, lining up a load for a flatbed truck. The employee had placed dunnage on the tire and moved it right as the load was put in place. This caused the trailer to level out and catch his finger between the dunnage and the trailer frame. He sustained a partial fingertip amputation, no bone, to the fourth left-hand digit.
