An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.

Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

An employee worked a shift and later experienced cramping in his arms and legs. He was hospitalized with dehydration.

An employee worked in the paper machine area for a shift, cutting corners off paper rolls around the roll machine. Afterwards, the employee sustained cramping and dehydration due to heat. The employee was hospitalized.

An employee was adjusting a trim sheet leading to a sheeter machine. Their glove became caught in the machine and the shearer closed on the employee's right hand, crushing it and breaking the hand and wrist.

An employee was troubleshooting a process upset. A nearby refiner had a pressure spike, causing the housing to separate slightly, which allowed the loss of primary containment. The employee was sprayed by hot water, resulting second- and third-degree burns on his torso and left arm.

On July 22, 2024, at 10:30 AM, an employee was responding to a pulp mill manufacturing process shutdown. Pulp stock overflowed through an engineered overflow pipe resulting in a buildup of stock in a bermed area. The stock was 140 to 150 degrees. The employee went to the bermed area to close an air-actuated process valve. While closing the valve, pulp stock entered the top of his boots and caused second-degree burns to both of his lower legs down to his feet. The employee was hospitalized.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
