An employee was using a large pot to carry hot oil from the fryers. His left foot got caught on a hose that was on the floor and the pot fell. When the pot hit the floor, the hot oil splashed upward onto the employee's face, hands, and legs causing second-degree burns.

An employee was walking from their work station when he stepped on a wooden board and rolled his ankle. He sustained a pelvic injury that required surgery.

While on a scissor lift, an employee was cleaning an overhead fan. The employee tripped over the hose of a wet/dry vacuum and his right hand struck the running fan blade resulting in amputation of the middle fingertip. The fan was not guarded at the time of the incident.

At 3:23 p.m. on March 21, 2022, an employee was returning to her workstation when she tripped over a plastic band with her left foot, suffered a broken right femur as she tried to regain her balance, and collapsed to the floor. She was hospitalized.

An employee was climbing down a track ladder. The employee's left foot twisted on a step as their other foot reached the floor, and the employee's shin was broken.

An employee was conducting a roof inspection for an insurance claim. He set up a ladder up on a deck and was climbing from the ladder to the roof when the ladder slipped causing the employee to fall to the ground. The employee was hospitalized with a fractured wrist and ankle and required surgery.

An employee was using a 16-foot ladder to descend from a second-story roof to a porch roof after completing an inspection. As the employee stepped back onto the ladder, they fell, landing on the porch roof. The employee then rolled off the porch roof and landed on their feet on the ground. They were hospitalized.

An employee was climbing from a ladder onto a residential roof to inspect it when he slipped off the roof and fell to the ground below. The employee's spine was broken.

An employee fell from a two-story roof to the ground while conducting an insurance inspection, suffering injuries to the head, neck, back, and left leg and a brain bleed.

An employee had just inspected hail damage on a one-story roof and was coming down a ladder to inspect a gutter. The employee fell from the ladder and broke both legs.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
