An employee was putting a chain on a barrel when their right index finger was caught between the chain and barrel, resulting in a partial amputation.

An employee was moving a piece of bar material inside a hydraulic vise on a band saw machine. The switch was toggled closed, crushing the employee's left middle finger in the vise. The finger was amputated.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was adjusting the tension on a grinder belt when three of their fingers got caught in the belt and pulley and were amputated.

His sleeve got caught in the conveyor and his right arm was pulled into the pulley. The employee was hospitalized with an open fracture and a crush injury/laceration to his right forearm. The machine was running at the time.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
