An employee was moving boxcars on a rail line in a plant when his jacket pocket was caught on a boxcar. He was then dragged approximately 20 yards over a pile of 4x4's and into a concrete post, injuring his left hip area.

An employee was loading a corn train and opening and closing the top hatches of the grain rail cars. As the employee unhooked the snap hook of their self-retracting lifeline (SRL) that was temporarily hooked to a rail car, the cars began to move. The employee's left index finger was caught between the snap hook and the rail car, resulting in partial amputation.

An employee was opening train car hatches on top of a train. The train was backing up to scan the cars in when the employee's foot became stuck. He was then dragged across two rail cars sustaining a concussion and a toe injury.

Gavilon Group LLC

Two employees were opening lids on railcars to prepare the cars for loading on a track. Both employees were in full body harnesses and were connected by a self-retracting lifeline (SRL) to a rail fall safety system. The train started moving north and the employees realized that the cars were going to move beyond the fall protection coverage and decided to unhook from the rail fall protection. Employee 1 assisted the injured employee and unhooked from the system. As the injured employee attempted to unhook, the SRL ran out of cable, roughly 25 feet from the rail shed. The injured employee was pulled onto his back and remained stationary on his back, held by the SRL cable, as the rail cars moved under him. The employee sustained a laceration and contusion to the top of the head; bruising and scrapes to the lower back, legs and arms; a cut on the right hand; and a bruised thumb.

Allied Universal Security

An employee was opening a train gate for a rail company. Before the employee could open the gate, the train struck the gate which knocked the employee down and rendered him unconscious. The employee sustained fractures to the skull and right eye socket.

An employee was riding on an amusement park's mountain ride. He lost his balance and came partly out of the cart; his arm hit the track and his arm was broken.

An employee was using a hydraulic bearing puller (held by a chain hoist) to remove a bearing from a railroad axle. The hydraulic bearing puller fell from the hoist (approximately 18 ) and struck the employees left foot, fracturing it.

The employee was climbing into the driver side of the truck in the parking lot while holding a lunchbox and a clipboard. The employee slipped on snow and fell to the ground, fracturing a hip.

R&L Carriers Shared Services, LLC

An employee was making a delivery and was pulling the customer's freight to the rear of the trailer using the plastic strap around the freight. The strap broke, causing the employee to fall from the rear of the trailer onto the asphalt pavement below. The employee sustained fractures to their left shoulder, elbow and pelvis.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

CHRISTUS St. Frances Cabrini Hospital

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

Jean Simpson Personnel Services, Inc.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
