An employee was inspecting his equipment to make sure all safety equipment was in place and properly operating. He was moving a flex lance to another tube when the dump valve was engaged. High-pressure water from the flex lance tip released and cut his left hand.
An employee was hydroblasting when the sprinkler head malfunctioned. Water shot out of the hose at 18,000 psi, blowing off a piece of pipe and tubing. The piece impaled an employee's left leg, breaking the tibia.
