An employee was using his hands to pull out one roll of underlayment roofing material from a middle pallet (in a stack of three) when the middle and top pallets fell down and knocked him on the floor. He fractured a foot and femur, requiring hospitalization.

He then slipped and fell, contacting metal flashing with his hand. His left hand was lacerated (cut through the tendon), requiring hospitalization.
