Final narrative
An employee was rigging a crane to pick up a piece of channel off the top shelf of a material rack. The employee was using a 4-foot step ladder. While stepping down, the employee missed a step and fell to the ground. The employee sustained a femoral neck fracture of the right hip and was hospitalized.
An employee was clamping material in place. The employee's fingertip was crushed between the material and the clamp, causing an open fracture and a soft-tissue amputation.
