An employee was coming out of a truck with a pneumatic nail gun. The gun fired a nail into his left foot, breaking a bone.

An employee was attempting to unclog a jammed vacuum unit when his right hand entered the energized unit. His right index finger was then caught between the vacuum's airlock and flange, resulting in a partial finger amputation.

An employee triggered a pressure washer while the nozzle was pressed against the top of the injured employee's boot. The water blast penetrated the injured employee's boot and cut the top of the foot.

An employee was performing maintenance on a material handling conveyor system when his right index finger was amputated above the first joint.

An employee was inside an intermodal container positioned to the right of granite slabs that were stacked in an A-frame manner. As a forklift was unloading the slabs from the left side of the A-frame, the slabs on the right side fell and struck the employee. The employee was pinned between 7 to 8 slabs and the side of the container. The slabs weigh approximately 700 pounds each. The employee sustained bruised ribs and lungs.

On December 30, 2023, an employee was pulling a pallet into the back room when a forklift rounded the corner and ran over the employee's right foot. Three of the employee's toes were amputated.

An employee was unwrapping a pallet of empty buckets. The employee stepped backward and was struck by a powered industrial truck carrying a pallet of bucket lids, resulting in a left ankle fracture.

An employee was walking in the racking area to check parts when they were struck by an oncoming forklift. The front wheel ran over the employee's right leg, resulting in fractures to the leg and ankle.

A tow truck operator was operating his bed controls on the drivers side when another vehicle crashed into him. The employee sustained fractures to the right femur, hip, and left foot, as well as abrasions to the right knee and a laceration to the right wrist.

An employee was cutting a piece of wood on the saw when the wood kicked back and struck the employee's stomach. The employee sustained internal bleeding in his stomach.

An employee was pushing a cart to move it when it collapsed on him. The employee was pinned underneath natural stone remnants that were on the cart, resulting in fractures, a dislocated hip, and a laceration to the area around his collarbone.

On June 3, 2025, at approximately 11:30 AM, an employee was retrieving a slab of stone when it fell and pinned both of his legs against the ground. The employee required surgery.

An employee was setting up the jobsite area to start trimming a stone countertop. He was using a saw when the blade broke and a piece of the blade went into his abdomen. The employee required surgery to remove it.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
