Final narrative
An employee was taking a wheeled garbage can outside to the dumpster when the wheeled base came off on the ground. The employee did not notice the wheeled base on the ground and stepped on it, which caused him to fall to the pavement and fracture his left hip. It was dark outside at the time of the incident.
On March 27, 2025, a truck driver had pulled up to a riser to load fuel into their truck. The driver was pulling down on the loading arm at the fuel terminal, or lifting the fuel loading arm when they felt pain in their abdomen. The employee sustained a hernia.
On January 18, 2025, at approximately 7:50 a.m., a field truck driver was picking up a hose that was connected to the truck to put it on the ground when he tripped and fell to the ground, fracturing his hip.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
