An employee was moving an electric buggy to gain access to an electric pallet jack. While operating the buggy in reverse, the employee was pinned between the buggy and a new loader tire resulting in a fractured left tibia and fibula.

A load of steel was being moved with a powered industrial truck. An employee was guiding the load while walking down a corridor when he lost his grip on the tagline and attempted to stabilize the load with his hand. The load then pinned his hand against the wall, fracturing it.
An employee was training on an rolling container carrier on the dry, non-frozen storage side of the distribution center. She made a turn around the end cap and her right foot struck the rail protecting the endcap racking. The employee's foot was fractured.
