An employee was troubleshooting a 3-phase electrical circuit and was replacing a fuse. An arc flash occurred and the employee suffered burns to both hands.

On May 26, 2021, an employee was operating the bread packaging machine near the conveyor and sustained the amputation of their left middle fingertip.

An employee was moving a mobile conveyor when it fell and landed on their foot fracturing it.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was operating a stretch wrap machine in the packaging area when his little finger was caught in the machine's chain mechanism. He sustained a fracture to the tuft of the finger and was hospitalized. The machine was unguarded at the time of the incident.

On August 12, 2025, an employee was performing maintenance on a motor and suffered an amputation to the left index fingertip, as well as a laceration to the left middle finger.

An employee was performing sanitation tasks and adjusting a valve by tapping it with a rubber mallet. The valve came apart and the employee was sprayed with 180-190-degree water. He was hospitalized with thermal burns to multiple parts of the body.

An employee was cleaning the rice cake machine. He then went to move the machine when it caught and amputated his thumb.

An employee was cleaning ovens from a three-step ladder. The ladder slipped, causing employee to lose balance and fall to the floor, suffering a broken left leg.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
