An employee was working in the rod basket on a well service rig when their left ring finger was caught in a pinch point between the rod basket swivel frame and a support on the rig. The finger was amputated.

On January 14, 2023, an employee was using a pipe wrench to free up a tube during a back-off procedure. The pipe wrench swung and struck the employee in the head, causing a contusion.

An employee was helping connect tubing on a hydrowalk. His middle finger was pinched between the tubing and the hydrowalk. The finger was amputated.

An employee was conducting maintenance on a pipe and was using a T-bar tool to secure the retainer cap on a pump. The employee's left index finger was caught in the rotating pump, resulting in a partial amputation.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
