An employee was manually flipping an 18" by 30" cross head guide that weighed 150 pounds when the part landed on his right hand, crushing his ring and pinky fingers. His pinky finger required stitches and his ring finger was amputated to the first joint.

At about 10:40 a.m. on March 16, 2021, an employee was pushing a 24-inch-long, 6-inch-diameter steel pipe with a forklift. The pipe slid off the fork, smashing employee's right index finger against the fork. The employee suffered a laceration to the finger.
Employees were replacing a 24" pig trap door. A strap was shackled on the pig trap door and connected to a forklift. Welders began to cut the door off. An employee was holding onto the door when the cut was finished. The door shifted and caught his fingers, resulting in a hand injury and amputation.
An employee was fitting an arbor fixture into a rotor housing to be reworked. The rotor is moved by a hoist. The arbor was being lowered into the rotor housing when the arbor slipped down onto his hand and amputated a portion of his left thumb.
