An employee was preparing girder #2 to be raised from the ground and put in position with girder #1. He was standing on top of girder #2, which was approximately 9 feet high, when girder #1 rolled over and fell. He heard the noise and jumped from girder #2 to avoid being struck. He fell and hit his head, suffering a left eye bruise and multiple facial fractures.

An employee was picking up a car from a fenced-in area when they became locked inside. The employee climbed the 7-foot fence, jumped off onto the concrete surface, and sustained bilateral ankle fractures.
An employee was descending an extension ladder that was set up on a cable strand at approximately 18 feet. When the employee was about halfway down, the ladder started to shift to the left and the employee jumped off the ladder to the right. The employee landed on a concrete sidewalk and their right ankle was fractured.
An employee was practicing an acrobatic act and jumping from one swing to another when she fell approximately 10 feet to the wood floor below, resulting in a lower lumbar fracture. Fall mats were in place at the time of the incident, but the employee fell outside of the mats.
