An employee was practicing an acrobatic act and jumping from one swing to another when she fell approximately 10 feet to the wood floor below, resulting in a lower lumbar fracture. Fall mats were in place at the time of the incident, but the employee fell outside of the mats.

An employee was picking up a car from a fenced-in area when they became locked inside. The employee climbed the 7-foot fence, jumped off onto the concrete surface, and sustained bilateral ankle fractures.

When the employee was about halfway down, the ladder started to shift to the left and the employee jumped off the ladder to the right.
