An employee was stacking 55-gallon drums when his middle finger was caught and lacerated in the pinch point between stacked drums. He was hospitalized.

When he opened the valve to initiate the pour, the valve came off, and hexamethylenediamine splashed on him, causing first and second degree chemical burns to his face/upper body.
