An employee was operating a pallet jack and got a foot stuck between the pallet jack and the wall. He suffered a broken ankle.
An employee driving a pallet jack entered the rail dock through the overhead door from the expansion dock. He began traveling east on the rail dock and the pallet jack ran into a barrier pole that protects a door on the rail dock. His foot was caught between the pallet jack and the barrier pole. He sustained a crushing injury to the left foot resulting in amputations to the tips of two toes. He was wearing safety toe boots at the time.
