Federal OSHA recorded a severe workplace injury at Georgia-Pacific Consumer Operations LLC, 1000 West Mount Pleasant Road, ZACHARY, LOUISIANA 70791 on — Second degree heat (thermal) burns, affecting the multiple body parts, n.e.c..

Two maintenance employees received second degree burns on their torso, legs and arms while troubleshooting a pump that moves ambient air, water and paper stock through a sewer/pipe system. The line/pump was plugged and the pump was shut down. After a period of time where the pump was running (not moving water and paper stock) and plugged, the ambient water and paper stock heated up. The maintenance personnel did not realize that the water had heated (temp unknown at time) and proceeded to open the inspection port. As the inspection port was opened, a sudden release of hot water/paper stock sprayed the two maintenance personnel.

The injured employee was engaged in a conversation with other employees. When the injured employee turned to walk away, she tripped over a pallet in the packing area and fell to the floor. The employee sustained a dislocated shoulder and a humeral head fracture.

An employee was using a shovel to remove waste vermiculite from molten zinc. The metal had been placed in a bin and partially hardened. The employee broke through the partially hardened metal; still-molten metal flowed to the employee's steel-toed right boot and entered through the cloth boot tongue. The employee suffered a third-degree burn to the right foot and was hospitalized.

Georgia-Pacific Palatka, LLC

Tools and parts were being lowered to the ground from an elevated platform in a 5-gallon bucket. The rope being used to lower the bucket failed; the tools and parts fell and lacerated the left forearm and upper left leg of an employee on the ground.

An employee was tracking an infeed belt on a scrap hogger when their right arm was caught between the belt and a roller. The arm was broken and the employee's shoulder was dislocated. The employee was hospitalized.
