On June 16, 2018, at approximately 9:00 a.m., an employee was attempting to check the gear box on top of a ready mix truck when the employee slipped, fell, and struck the truck fender and the floor. The employee suffered three broken ribs and a punctured lung.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
