An employee was lancing heater tubes with pressurized water equipment. When he pulled the lance out, the pressurized water struck his left bicep and chest, resulting in abrasions and hospitalization.

An employee was fueling a gasoline-powered pressure washer with a safety fuel can. During the transfer the fuel ignited, causing a fire. The employee sustained burns to his nose, and his right hand and forearm.
An employee was using a pry bar to remove a component from a water pump when the component shifted and the pry bar pinned his finger against the pump. The employee sustained a lacerated and broken left ring finger.
An employee was loading and unloading cartridges with a forklift. While disengaging the hopper, his left hand slipped and was crushed by the hopper, resulting in a laceration to his left third digit and an amputation to the tip of his left second digit.
An employee was standing approximately 20 feet away from a coworker that was hooking up a 6,000 psi sewer jet to clean out a pipeline. When the hose was turned on in the truck, the hose came out of the pipeline and sprayed the employee's chest just below the breast line. The employee was cut and hospitalized.
Employees were cleaning a tank of Green Liquor with a fire hose. They started from outside the tank and were washing the chemicals toward the drain with a high volume of water. The Green Liquor was around the drain when two of the employees entered the tank. They were wearing chemical protective suits with knee high boots that were taped around the top. As the employees moved toward the drain, the Green Liquor flowed around behind them and rose above their boots. The Green Liquor seeped through the tape and burned the employees' legs. One employee did not require medical treatment. The second employee was hospitalized.
An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.
