Final narrative
An asphalt roller operator fell approximately five feet from the operator's platform and was hospitalized for a severe head injury with concussion. The roller was parked at the time.
An employee was rotating a vibratory hammer to extract steel sheeting when his right ring and little fingers got pinched between the hammer and a stationary pipe that was coming up out of the ground next to the steel sheeting. The employee suffered a crush injury to his ring and little fingers as well as a partial amputation of the ring finger.
On October 18, 2023, an employee was refueling a dozer while standing on its treads. The dozer moved forward and the employee fell off the treads toward the back of the equipment and landed on the ground sustaining a fractured left leg and kneecap.
On April 7, 2023, at approximately 9:30 AM, an employee finished removing a coupler and was between a the boom and stationary digging bucket on the ground. The excavator activated and swung toward the bucket. The employee sustained a crushed pelvis.
On January 28, 2021, a crane block and hook had been lowered to the ground for removal from the crane. As the crane load hook came in contact with the ground, an employee tried to rotate the load hook so the block/sheaves would come to rest in a vertical position on the ground.
