An employee was gathering boxes with a pharmacist in the warehouse. She stepped backwards, tripped, and fell over the corner of a pallet fracturing her left wrist.
An employee was clipping paper on a white board. When she turned away from the board, she tripped on the leg of the white board and fell striking her left knee on the floor and her right knee popped. She felt pain in the right knee and had difficulty lifting herself from the floor. She was hospitalized overnight with a sprain, strain injury.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was assisting with maintenance and servicing a conveyor that was making an unusual noise. The employee's right arm got caught in the conveyor between a belt and a roller. Their right hand and arm were crushed, resulting in amputation of the arm.
