An employee was clearing gypsum from a pump trailer machine's hopper while standing on the machine's auger. The machine started unexpectedly and twisted his right foot into the opening below the auger. His first toe was badly damaged, the second was partially cut off, and the third was lacerated. The machine was off but had been switched to an automatic setting at the time of the incident.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
