An employee was using a die changeover table with a snake chain conveyor. The e employee activated the snake chain via remote and it moved quickly, causing the employee's right index finger to be pinched between the pin and edge of the die and amputated.

WHIRLPOOL CORPORATION

On January 28, 2019, at approximately 4:35 p.m., an employee was attempting to move a plastic banded pallet in the bed of a pickup truck when the strap broke causing the employee to fall backwards out of the truck onto the concrete floor. The employee sustained head and neck trauma.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
