Final narrative
An employee was operating a walk-behind pallet jack when he bumped a stack of pallets with his left arm, causing him to lose his balance. He then fell and fractured his right hip.
An employee was travelling into the cooler in the shipping area while operating a pallet jack. He tripped over the forks of a parked jack and the pallet jack he was operating then ran over his right foot. The employee sustained fractured toes.
An employee was cleaning pieces of meat from a poultry skinner. Her hand was caught in the machine, two opposing rubber belts (which rub skin off the meat as it enters the machine) caused an amputation of her left little fingertip, broke the tip of the bone, and degloved the palm side of the finger.
The employee reached into a poultry skinning machine to push meat through. The metal mesh glove she was wearing became caught and pulled her hand into the machine, amputating her middle fingertip and nail.
