An employee was winding rolls of sorbent plastic material when one of the cores became stuck. She hit the core with the palm of her hand as instructed to make it move again. It began rolling and her left hand was caught and crushed in the machine. She suffered bruising and a fracture to the middle finger, a cut to the end of the middle finger that needed stitches, and some bone chipping to the pinky finger.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
