An employee was performing maintenance on a stamping press that had locked up. The employee was using a hydraulic jack and a make-shift tool to push the frame upwards. The pressure from the jack released causing the employee to be struck and thrown backwards about 10 feet. The employee's arm was also caught. The employee sustained broken bones.

The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was holding a piece of rubber while it was being cut on a band saw. The saw blade amputated his right middle fingertip without bone loss.

An employee was observing expansion rods on a coil expander going into a coil to ensure copper stubs weren't being bent. His left hand was caught between the locking block and locking block clamp on the power press machine, resulting in partial amputation of his left little finger from the distal interphalangeal joint.

An employee was setting up a press brake and rolling over a die when the tip of their left little finger was caught between the horizontal working surface of the press brake and die. The employee sustained partial amputation of their fingertip.

An employee was working to remove an access panel to begin performing maintenance on a machine. The set pins on the bottom of the panel were broken. The bottom kicked out and fell, striking the employee's left foot and resulting in a fracture to the big toe.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.
