E-J ELECTRIC INSTALLATION CO.

An employee was offloading portable power distribution unit (PDU) equipment from a delivery truck. The PDU was being lowered on the box truck's tailgate lift when one of the PDU's wheels rolled forward. The unit fell to the ground, toppled over, and struck the employee. The employee was hospitalized with injuries to the right side of their body including their shoulder and hip, and injuries to their left elbow. The employee's hip was fractured.

An employee was setting up stage equipment when they were struck from behind by a mobile cart, resulting in a fracture to their lower right leg.

An additional 480V service for a step-up transformer was being installed. When the circuit was tested, an arc flash occurred, resulting in lacerations to the employee's hand.

Employee used a derrick with a tong to unload a utility pole out of a storage area on the ground. The pole rolled onto the employee's leg as he tried to reorient the tong. The left fibula was broken.

Two employees were walking through the warehouse behind a forklift that was moving pallets of 30-foot metal bottom bars. When the forklift reversed, the top of the forklift struck a box of metal bottom bars on top of racking, causing the box to fall and strike the two employees. They were both taken to the hospital; one employee was treated and released without being admitted. The other employee was hospitalized with a fractured left orbital bone and an injured right knee.

The injured employee was spray painting lay flat shipping reels while another employee was picking up the reels with a telehandler after they were painted. The telehandler bumped a reel, causing other reels to fall. The injured employee was caught between the reels, resulting in broken left femur.

An employee was installing an aerial drop cable across a roadway. The employee was on a ladder positioned against a telephone pole. Cable had been installed on one side of the roadway. The employee was making the connection to the pole on the opposite side of the roadway when a box truck contacted the cable, causing the employee to fall onto the grass of the customer's yard. The employee sustained broken ribs and was hospitalized.

An employee was performing routine housekeeping duties when a bale of plastic bottles slid off the forks of a forklift that was backing out of a trailer. The bale struck the employee, pushing her into another machine. The employee suffered a fractured rib and punctured lung.

An employee was using a 4-by-4 to straighten a pack of wood. A forklift nudged the 4-by-4, which caused the employee's right hand to be caught between it and the pack of wood. The employee suffered a partial amputation to the right ring finger.

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was standing on an A-frame ladder approximately 6 feet above the floor while he attached a suspended light fixture to the ceiling. The employee lost balance, and fell to the tiled floor below, sustaining blunt force trauma to his head.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

The injured employee was performing routine maintenance on a fan belt/pulley. The injured employee and another person were moving the pulley back and forth manually to find a bearing issue. The injured employee's left ring finger got caught between the belt and the pulley wheel, resulting in a soft tissue amputation.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
