Final narrative
An employee was attempting to adjust a small pulley belt inside equipment when his left hand was pulled into the chain and sprocket, amputating his left pinky finger (below the middle knuckle) and left ring finger (above middle knuckle). The equipment was live at the time of the incident.
An employee was resetting the chain at the core station. He stepped onto the foot pedal that raises and lowers the hoist as he grabbed the chain and sustained amputation of his left index and ring fingers.
