An employee was cleaning the blade to a microtome machine when it was activated via the foot pedal. The blade then amputated the employee's left fingertip. The machine was energized at the time of the incident.

An employee was responding to a code for aggressive behavior from a patient. Upon stepping out of the elevator, the patient attacked the employee and the employee sustained hand lacerations.

An employee was working in rooms along a hospital hallway, performing respiratory treatments for her assigned patients. She noticed a bleach smell in the hallway as she entered a room. While in the room she continued smelling bleach, so she put on a surgical mask and closed the door behind her. Later, after working in other rooms and removing her mask, she entered a room with a recliner that had been wiped down with bleach shortly before she entered. She was hospitalized, suffering respiratory distress.

The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.
