An employee was observing a forklift under operation when the boards shifted and his right hand became pinched between the supports of the forklift attachment and the forklift tine. The employee sustained a fingertip amputation.

The injured employee was working on the concrete block line. The line had stopped while maintenance was tracking the conveyor belt to center it on the rollers. The injured employee attempted to secure a guide to help final product onto the conveyor and his right hand became caught between the head pulley and was pulled down into the machine. His index and middle fingers were amputated.

An employee was building 10-to-12-foot trusses on a truss production line. When the trusses are finished, a conveyor arm lifts up the conveyor so that a forklift can remove the trusses from the conveyor. Once the forklift is in position, employees have to lower the arms manually to allow the forklift to back out with the trusses. When the employee went to lower the arm, it came down quickly and pinched his right index finger between the framework of the conveyor and the bottom part of the arm. The employee suffered crush injuries that required surgery.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up.
