While instructing a new employee, the injured employee was struck by a triple pallet jack and sustained a fractured ankle.

On January 11, 2021, an employee's left leg was caught between a freight elevator and the wall of the elevator shaft. The employee suffered broken bones and soft tissue damage and was hospitalized, requiring surgery.

On June 6, 2019, an employee traveling on a forklift struck a rack, had a seizure, fell off the forklift and struck his head on the floor, fracturing his eye socket.

An employee was setting up to put tires on a trailer. The trailer was jacked up and he was putting jack stands under the front axle when the trailer bags released and the trailer dropped down, crushing his right index finger between the axle and the jack stand. His fingertip was amputated.

An employee was taking an electric walking jack out of a trailer when the electric jack, the employee, and the dock plate fell to the ground. The employee suffered a broken leg, requiring surgery.

On December 26, 2023, an employee went out to the parking lot to troubleshoot the starter of an SUV. The employee was underneath the vehicle when it started and moved and the employee's middle back area was caught under the front passenger-side tire. The employee sustained a fractured collar bone.

An employee was parking at a patient's home and exited the van to manually open the garage door. The van began rolling backward and rolled over the employee's right leg. She was hospitalized with a fractured leg.

A driver exited their truck so they could provide service for a customer when the truck began to roll away. The driver went to re-enter the truck but fell and was struck by the rear driver-side tire. The employee sustained fractures to their leg and foot.

An employee had been operating a yard tractor. He parked the vehicle and was exiting when the vehicle began to roll, causing him to fall. The employee's hand was then run over by the yard tractor, resulting in fractures to multiple fingers. The employee was hospitalized.

An employee was checking for a leak in the air brake system of a semi-truck. The employee had removed the brake chamber when the truck rolled back. One of the tires pinned the employee's right hip against the ground.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

An employee was operating an electric pallet jack when it pinned his right ankle against some racking. The ankle was broken and he was hospitalized.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
