In the back of the warehouse, two temporary employees were unloading a truck that was full of bags. The bags tipped over and fell onto one of the employees and fractured his legs. The employee required surgery.

A temporary employee was on the second rung of a ladder installing a window on the third floor of a residence. The suction cups on the window failed causing him to fall approximately 10 feet. He fell through the scaffolding on the second floor and then rolled down the stairwell to the first floor. The employee sustained fractured discs and a fractured hip.

A temporary employee was flagging traffic on a road construction site at night. A car struck the employee, who suffered a broken right arm.

An employee was struck and injured by a forklift. The employee was hospitalized.

At about 2:30 p.m. on June 15, 2020, a temporary employee was drilling holes in the outside wall of a house (to install shutters) when he became lightheaded due to heat exhaustion and dehydration. He was hospitalized.

An employee was loading a dish washing machine. A plate bumped against the machine and broke, lacerating the employee's left arm. The employee was hospitalized and required surgery.

An employee was throwing a mirror into a dumpster when the mirror shattered and lacerated the employee's arm.

Two Rycon employees were installing glass marker boards in a nearly completed space. Each employee had a suction cup on the face of the board and a hand on the bottom of the board. Upon hanging the third board in sequence on a hang strip mounted to the wall, the board needed to be moved about half an inch to butt tightly to the adjacent one. When the employees moved the marker board, it shattered causing half of the board to fall and cause a severe laceration to an employee's left wrist.

On May 15, 2023, an employee removed a mirror from the bathroom to discard it. As the employee went to place it on the cart, the mirror broke. The employee sustained a laceration on the right forearm that required surgery.

An employee was lifting a round, glass table top while working on a waste collection route. The glass table top shattered and lacerated the employee's arm.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

A temporary employee was rigging secondary steel when the steel joist fell to the ground and struck his lower left leg, resulting in fractures to the tibia and fibula.

A temporary employee was helping set up a stage for a concert. The employee was run over by a mobile cart and sustained a toe amputation. The employee was hospitalized.

The injured employee was operating a reach truck and stopped at an intersection within a warehouse. After a brief a conversation, another employee's reach truck accelerated unexpectedly and struck the injured employee's stationary truck crushing his foot. He sustained fractures to his left foot and toes requiring hospitalization and surgery.

A temporary employee was operating a plastic strapping machine on an assembly line. The machine wrapped the employee, causing crushing injuries to their ribs and a kidney. The employee was hospitalized.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
