A metal tote was being carried on a forklift. The tote shifted forward during a turn, slid off the forks, and struck an employee standing below. The employee suffered a fractured pelvis and a fractured knee.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

She landed partially in the doorway of the dictation room and partially in the hallway. She struck the ground with her right arm and shoulder. She experienced numbness and sustained a gash above her left eye that required stitches. She was hospitalized with a dislocated and fractured right shoulder that required surgery, a dislocated right elbow, and injuries to her head and neck.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.
