Employees were using a machine to remove a phone pole on the street. After the pole was pulled from the ground, an employee's left middle finger was crushed between the pole and the concrete. The fingertip was amputated.
An employee was putting a manhole cover in place. It caught the employee's right middle finger as it seated itself, causing the amputation of the fingertip.
