An employee was operating her machine from the box packing area when a coworker asked where to place a tape gun and she pointed with her left arm to the location. At the same time, an air cylinder retracted and caught her left arm, causing an elbow dislocation, laceration, and fractured wrist.
On April 27, 2022, an employee was operating a band sealer when its outfeed jammed. She was clearing it when the jammed piece pulled her into the machine; this triggered the machine to actuate, and a knife in it partially amputated her index and ring fingertips without bone loss.
