The employee got off the pallet jack while a co-worker used their pallet jack forks to lift up the vehicle so the employee could remove the plastic. As the employee grabbed the plastic, the pallet jack shifted off the co-workers pallet jack and fell onto the employees' fingers. They sustained amputations to the right middle and ring fingertips.

The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
