Final narrative
An employee was operating a powered pallet jack when he noticed plastic was wrapped around the wheel. The employee got off the pallet jack while a co-worker used their pallet jack forks to lift up the vehicle so the employee could remove the plastic. As the employee grabbed the plastic, the pallet jack shifted off the co-workers pallet jack and fell onto the employees' fingers. They sustained amputations to the right middle and ring fingertips.
An employee was reaching a shelf in the storage room when he lost his balance and fell to the floor. The employee was hospitalized with four fractured ribs, two on each side.
