An employee was on the second step of a step ladder, using a box wrench to tighten a bolt on a pipe flange. The wrench slipped off the bolt and the employee fell backward, striking his head on a toolbox and losing consciousness.
On September 11, 2023, an employee was standing on a ladder and working to fix a leaking pressure valve. Beer was released from the valve and got on the employee causing them to slip and fall from the ladder to the floor. The employee sustained a fractured left humerus.
A maintenance employee was helping to troubleshoot a fault in the hoist on a bottle line de-palletizer. A broken sprocket needed to be removed in order to level the hoist. The machine was locked out/tagged out and the cover of the chain and sprocket was then removed.
