On September 8, 2022, an employee was working with cinnamic aldehyde and noticed his arms had some irritation after leaving work for the night. After showering, the irritation became more intense and a rash appeared. The employee was hospitalized for a rash on both arms between the elbow and the wrist.

An employee was cleaning a pump. While his finger was in the pump's discharge port, the end of his right middle finger was amputated.

On December 19, 2023, an employee was setting up a copper wire drawing machine when his left ring finger was pinched between a copper wire and a machine part. The employee's finger was partially amputated.

An employee was examining a machine when a part moved up and amputated a fingertip on their left hand.

An employee was checking the gusset seal on a machine. He moved to the next cross sealer and the machine activated, causing an amputation to a left-hand finger. The employee was hospitalized.

An employee was working on a valve replacement when their left middle finger became caught in the butterfly flapper of the valve, resulting in a fingertip amputation.

An employee was walking when they were struck by a forklift. The employee's leg was caught between the forklift and a storage rack, resulting in a leg injury that required hospitalization.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
