Federal OSHA recorded a severe workplace injury at STEPHENS & SMITH CONSTRUCTION CO., INC., 5711 S. 60th St., OMAHA, NEBRASKA 68117 on — Fractures, affecting the chest, except internal location of diseases or disorders.

Employees were pumping concrete into a 6-foot wall. The pump was plugged, then surged, causing concrete to spray out of it in force and the hose to whip. One of the employees was struck by the hose and knocked off the wall to the ground. The employee suffered broken ribs.

An employee was hooking down a power trowel to the attachment on a forklift when the soft ground under the forklift shifted and caused it to move. This made the power trowel lunge back and catch the employee's right arm between the power trowel and the forklift, resulting in a laceration on his forearm.

An employee was sitting on a stool while working on a trailer and stood up to reposition the stool. At the same time, the stool's cushion fell off, exposing the cylinder of the chair. The employee lost balance and fell onto the cylinder of the chair, causing abrasions and lacerations to his abdomen.

An employee was working with a crew to lift a gang form with a forklift/tractor over an existing portion of gang form. The employee was on the inside corner of the wall portion of the forms where they were connected on the ground. The tag line caught on the top corner as the form was coming over the existing wall, causing the gang form to rock; the scaffold plank came loose and struck the employee. He fell to the ground, suffering head lacerations, broken ribs, and a possible back injury.

Three employees were removing pins from gang forms in order to lower a form. When the upper pin was removed, the form fell forward, striking and landing on top of the employees. One employee received serious back injuries, while another employee suffered contusions to his upper body and a broken left ankle. They were both hospitalized.

An employee was getting into a footing approximately 3.5 feet deep to show a crew member how to tie the rebar. As he entered the footing, his foot slipped and fell on the rebar. He suffered a laceration/puncture wound to the base of his neck next to the clavicle.

InterCon Construction, Inc.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

RAVA Construction, LLC

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

Stein, LLC

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

BRUNDAGE-BONE CONCRETE PUMPING, INC.

An employee was changing a boom pipe delivery system on a concrete pump truck. He tripped and fell from an elevated level to the ground, resulting in multiple leg fractures.

An employee was reading numbers off a belt that feeds a plant. He reached his hand up and the belt caught his finger, resulting in a finger amputation.

Paul Conard Construction Co., Inc

An employee was guiding the pouring end of a concrete pump truck's boom while standing on top of 4-foot wall forms. The boom contacted power lines and the employee was shocked. The employee sustained third-degree burns on the entry and exit path of the electricity, and also sustained first- and second-degree burns to their torso and legs.

An employee was holding a form board on a section of pavement. A skid steer backed into the employee's ankle, breaking it. The employee was hospitalized.

Concrete Industries Nebraska City

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

Elkhorn Mechanical and Combustion

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

Cellar Crew LLC

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
