An employee was working on a main distribution panel (MDP) when an electric shock resulted in burns that required hospitalization.

An employee was laying steel decking on a roof. The employee fell to the concrete floor about 10 feet below, suffering a head injury and facial fractures.

An employee was cutting the edge off a 2-by-4 using a handheld circular saw. The guard caught and the saw cut the employee's left thumb, breaking a metacarpal and causing arterial bleeding.

A temporary employee backed into a parked cement truck while operating a concrete buggy, resulting in contusions and lacerations to the right leg.

On November 9, 2023, an employee was removing plywood from the west face of a basement level buck hoist to prepare to dismantle one of the cabs. When the employee pulled on the plywood, it fell and pulled a rope that partially amputated the employee's thumb.

An employee was holding a rigging line and assisting in the removal of an old light pole from its base. When the load was lifted by a forklift, it shifted. The tag line lifted the employee off the ground and propelled him through the air. The employee landed on the ground and sustained back fractures.

An employee was performing maintenance on a bucket truck when his right hand became caught in some rope, causing amputations to three fingers down to the first knuckle.

An employee was conducting a fall rescue demonstration. A 165-pound demonstration dummy was released from a height, putting tension on the lanyard. The employee's left hand, little finger, and thumb were caught in the wire rope lanyard, resulting in a medical amputation of the left little finger as well as lacerations to the left thumb.

An employee was using a crank and cable to lower a light tower due to windy conditions. The light shaft became hung up and released the tension in the cable, which wrapped around the employee's left hand, resulting in amputations of three fingers.

An employee was helping to install a tarp on a roof when they fell through a skylight to the ground below, resulting in fractures to their face and ribs.

On August 5, 2025, an employee was exiting a room when his left middle and ring fingers became caught in the door as it shut. The middle fingertip was amputated.

An employee was driving a forklift and unloading racks of glass from a truck. A rack began to tilt; the employee exited the forklift and went to stabilize it. The rack tipped and pinned his left arm against the forklift. The forearm was broken and required surgery.

An employee was sitting on a stack of steel beams. When they stood up, the top beam fell onto the employee, resulting in a broken left leg.

An employee was working to tow a forklift with another forklift when the tow line came loose and struck their face. The employee sustained head, face, and eye injuries.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
