An employee was operating a band saw when his right little fingertip was amputated.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was inspecting the attic in a home. The employee stepped off the drop-down attic ladder onto drywall, fell through, and landed on the floor about 10 feet below. He suffered a broken left tibia.

An employee went aloft in a bucket truck to install a rubber cover-up for the pole set. He was proceeding to frame and transfer the conductors to the new pole when he sustained electrical burns on his left side, arm, and leg that required hospitalization.

An employee was positioned on a scaffold. They were using a level to adjust a pole and place bricks on the side of a wall. They fell approximately 30 feet from the scaffold to the ground, resulting in fractures to their back, rib, and heel.

The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
