An employee was checking the alignment and tightness of ventilation system drive belts. A belt snagged the employee's glove, pulling the employee's hand into the machinery. The employee suffered multiple fractures, including an open fracture/fingertip amputation.
An employee was assisting with crane operations for a crane that was lifting a 20,000 pound shipping container. Something shifted and the container began to sway. The employee went to steady the load when his left little and ring finger were pinched between the container and the crane outrigger, resulting in fractures.
