An employee was on a forklift loading/unloading trailers when the trailer he was on pulled away from the dock door. The forklift with the employee strapped in fell four feet and landed upright. The employee suffered a fractured vertebra.

An employee was making a delivery and lowering the lift gate on the back of the truck when he tripped and fell from the lift gate breaking the left tibia.

On October 25, 2023, an employee was in the warehouse driving a forklift toward the dock door. The forklift struck a conveyor belt and the employee sustained broken ribs.

An employee was operating an electric pallet jack when it struck a stanchion. The employee sustained fractures to the right chest and right ankle.

An employee was operating a forklift when the vehicle collided with a wall. His right foot struck the wall, resulting in three fractures in his foot.

An employee was using a walkie-rider to load a trailer. As she turned a corner, the walkie-rider struck the trailer and the employee was thrown to the ground. She was hospitalized with a fractured left leg.

An employee was driving a forklift and struck a pole, resulting in a broken left ankle.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
