An employee was on a scissor lift while marking a ceiling with a tool so they could cut control joints. The counterweight of a boom lift struck the scissor lift as it rotated, knocking the scissor lift over. The employee fell 14 feet. The employee sustained a femoral neck fracture to their left femur, fractures to three vertebrae, a pelvic fracture, a fracture and dislocation to their left wrist, and scrapes to their left elbow and wrist. The employee was hospitalized.

An employee was installing insulation while on a ladder. He fell approximately 8 feet to the floor and fractured his right femur.

An employee was cleaning coal build-up inside pulverizers that were 10 feet 3 inches wide and contained three stationary rollers. After dislodging a piece of coal, the employee exited the pulverizers when the converging floor moved and their left leg was caught between one of the three rollers. The motor to the converging floor was not in place at the time of the incident.
