New Thermo Serv, LTD

An employee was operating a spin welding machine. She placed material into the welder, pushed both start buttons, and the machine cycled. The employee reached into the machine to remove the material and the machine cycled again, smashing the employee's fingers on her right hand (middle, ring and pinky fingers).

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

The employee's left hand was inside the hopper's butterfly valve when it unexpectedly turned. The employee's index and middle fingertips were partially amputated.
